Skilled nursing facilities (SNFs) are for patients who require long-term skilled nursing services as well as those who need short-term rehabilitation care.
How This Type of Care Fits Into Rehabilitation
SNFs offer short-term care for patients recovering from an illness or injury, as well as long-term care for those who no longer can stay at home. A wide range of rehabilitation services are offered at SNFs and are customized to meet each patient’s needs.
For short-term patients, the goal is to be able to return home and perform tasks necessary for everyday living. For long-term patients, the goal is to keep patients as safe and healthy as possible while enhancing the quality of life for each person to whom the facility is home.
Admission to an SNF
Patients needing short-term rehabilitation usually are admitted to an SNF right from a hospital, after a hospital stay for an illness or injury. For those who will be living permanently at a skilled nursing facility, the move is usually arranged by the person and/or his or her family, together with the admissions staff at the facility.
Upon admission, a personal care plan is developed by the facility’s interdisciplinary team along with a guide to how the care will be provided. This plan is monitored and modified as necessary throughout the person’s stay, whether short- or long-term.
