| | | Pain Medicine Fellowship | | General Information The Pain Medicine Fellowship of the Spaulding Rehabilitation Hospital (SRH) and the Harvard Department of Physical Medicine and Rehabilitation provides an integrated, multidisciplinary pain management curriculum, which is accredited by the Accreditation Council for Graduate Medical Education (ACGME). The fellowship program is designed to meet the objectives of the International Association for the Study of Pain (IASP), as well as Pain Medicine subspecialty requirements as dictated by the combined boards of The American Board of Physical Medicine and Rehabilitation (ABPMR), The American Board of Psychiatry and Neurology (ABPN) and The American Board of Anesthesiology (ABA). It is anticipated that upon completion of their fellowship, fellow(s) will be prepared to take the certifying Pain Medicine subspecialty examination offered by the respective boards of ABPMR, ABA and ABPN.Performance of EMG/NCS will not be part of the fellowship experience. | Time frame | | This is a 12-month fellowship, which commences on July 1st and ends on June 30th of the following year. Presently, only one fellow per year is accepted into the program. The fellow will be eligible for an academic appointment at Harvard Medical School through the Department of Physical Medicine and Rehabilitation. | | Scope | | Approximately 50% of the fellow(s)'s time is spent evaluating and treating patients in one of the Spaulding Rehabilitation Hospital clinics. The types of clinics include: headache, complex chronic pain, spine and general musculoskeletal. The fellow also plays an integral role in evaluation of patients for participation in the outpatient multidisciplinary pain rehabilitation program, which consists of Physical Therapy, Occupational Therapy and Pain Psychology. Other responsibilities include evaluation and treatment of patients in the Spine Center at Newton-Wellesley Hospital as well as working with a Pain Medicine Board-Certified anesthesiologist in a private office community setting. Additionally, participation in an off-site cadaveric course for implantable devices is offered. Upon completion of the fellowship, the fellow will have acquired skills in performing the following interventional procedures: | • | Interlaminar epidural steroid injections (cervical, thoracic, lumbar) | | • | Transforaminal epidural steroid injectins (cervical, thoracic, lumbar, sacral) | | • | Caudal epidural steroid injections | | • | Intraarticular facet joint injections (cervical, thoracic, lumbar) | | • | Diagnostic medial branch blocks (cervical, thoracic, lumbar) | | • | Sacroiliac joint injections | | • | Intraarticular hip joint injections | | • | Diagnostic provocative discography | | • | Stellate ganglion blocks | | • | Lumbar sympathetic blocks | | • | Superior hypogastric ganglion blocks | | • | Impar ganglion blocks | | • | Intercostal nerve blocks | | • | Epidurolysis of adhesions | | • | Radiofrequency ablation of facet joints (cervical, thoracic, lumbar) | | • | Spinal cord stimulator trial/implantation | | • | Intrathecal pump trial/implantation |

Ali Mosutoufi, MD Residency: Physical Medicine and Rehabilitation - Tufts University School of Medicine Conditions In general, we expect fellow(s) to be in the hospital from 8:00 AM to 5:00 PM on weekdays, excluding official hospital holidays. Anticipated or unanticipated absences for medical or professional reasons should be discussed with the Program Director. Vacation time, four weeks per academic year, should be arranged well in advance and approved by the Program Director. Vacation time authorization is dependent on program needs. Unless specific approval is obtained, vacations should not be taken in increments greater than one week at a time. Additional responsibilities and conditions of the Pain Management Fellowship include: | • | The fellow will attend weekly didactics sponsored by SRH and will be responsible for online CME activities. | | • | The fellow will be responsible for covering telephone pages during the weeknight afterhours, on weekends and holidays, unless prior arrangements have been made. There are no in-house responsibilities on weeknights or weekends/holidays. | | • | It is expected that the fellow will participate in clinical research trials and other research projects during the course of the year. |
