We offer a wide range of rehabilitation services and nurse-administered medical care at our New England area Skilled Nursing Facility. We customize your care based on your individual needs and wellness goals by taking into consideration the complexity of your illness or injury and treating you as whole person. Our staff is specially trained to support both your physical and emotional well-being as you take steps toward improved health and function.

Upon admission, our clinical team will develop a personal care plan. This plan is monitored and modified as necessary throughout your stay, whether short- or long-term. For short-term patients, the goal is to be able to return home and to improve your strength and ability to perform tasks necessary for everyday living so that you can return home. For long-term patients, the goal is to keep you as safe and healthy as possible while enhancing your quality of life.

Admission to a Skilled Nursing Facility

Patients needing short-term rehabilitation are usually admitted to an SNF right from the hospital, after an illness or injury. If you will be living permanently at our skilled nursing facility, we are here to help you and your family arrange the transition.
