Skilled Nursing Facility
If you’re recovering from an illness or injury skilled nursing facilities (SNFs) offer short-term care, therapy and medical treatment for those who no longer require intensive care but are not yet ready to return home.
Our approach to skilled nursing
We offer a wide range of rehabilitation services and nurse-administered medical care at our New England area Skilled Nursing Facility. We customize your care based on your individual needs and wellness goals by taking into consideration the complexity of your illness or injury and treating you as whole person. Our staff is specially trained to support both your physical and emotional well-being as you take steps toward improved health and function.
Upon admission, our clinical team will develop a personal care plan. This plan is monitored and modified as necessary throughout your stay, whether short- or long-term. For short-term patients, the goal is to be able to return home and to improve your strength and ability to perform tasks necessary for everyday living so that you can return home. For long-term patients, the goal is to keep you as safe and healthy as possible while enhancing your quality of life.
