Our Case Management Department and Clinical Social Work services are here to give you and your family the best experience possible at every stage of care. It’s our job to focus on your care coordination so you can focus on you and your recovery.
Your case manager lets you focus on your care
Our dedicated case managers are here to help you and your family navigate the healthcare system. They will help you coordinate between different doctors across the care network and connect you with all the services and resources you need to reach your highest potential.
Case manager responsibilities include:
- Care coordination during your hospital stay
- Patient and family goals setting and communication across your care team
- Discharge planning, which begins early in your stay to establish a clear rehabilitation plan
While the ultimate goal is to get you home, some patients leave our hospital and continue therapy in another facility, sometimes called an acute care or skilled nursing facility (SNF). Your case manager will also coordinate this process if you need it. In addition, he or she will speak with your insurance company to determine financial coverage for your hospital stay and for post-hospital care.

Your clinical social worker will support your overall well being
Clinical Social Workers are responsible for addressing your social, emotional and interpersonal needs while you focus on recovery. They will help you address and adjust to the changes a new illness, condition or loss of ability can introduce into your family. Your Clinical Social Worker will also help you and your family plan for long-term healthcare management needs, including referrals to community resources and other opportunities that may promote your highest level of independence.
